India is quietly undergoing one of the most significant demographic shifts in its history. The country, long defined by its youthful population, is now ageing at a pace that catches policymakers, families, and healthcare systems off guard. Census data from 1991, 2001, and 2011 offers a clear window into this transformation, revealing not just rising numbers of elderly citizens but profound changes in dependency patterns, median age, and gender composition. Understanding these trends is essential because they shape everything from pension schemes to hospital infrastructure to the way families organise care.

What the census tells us about ageing
The Census of India is the most comprehensive source of population data the country possesses. It counts every individual once a decade and breaks the population down by age, sex, residence, and a host of other characteristics. For demographers studying ageing, the censuses of 1991, 2001, and 2011 form a crucial three-decade arc that captures the early stages of India’s demographic transition.
In 1991, the elderly population (defined in India as persons aged 60 and above) stood at roughly 57 million, accounting for about 6.7% of the total population. By 2001, this figure had grown to approximately 70.6 million. The 2011 census recorded a further jump, with the share of elderly reaching 8.6% of the total population, or about 104 million people. What stands out is not just the absolute increase but the acceleration. The decadal growth rate of the elderly population between 2001 and 2011 actually rose for the first time since Independence, signalling that the pace of ageing was speeding up rather than stabilising.
Why the elderly share is climbing
Two forces drive this shift. First, fertility rates have fallen sharply, meaning fewer children are being added to the base of the population pyramid. Second, life expectancy has improved due to better healthcare, nutrition, and disease control. The National Family Health Survey-5 confirmed that India’s total fertility rate has dropped to 2.0, below the replacement level of 2.1. As fewer young people enter the population while older citizens live longer, the elderly proportion inevitably rises.
The dependency ratio and what it means for policy
The dependency ratio is a deceptively simple but powerful indicator. It measures the number of dependents, typically children below 15 and adults aged 60 or 65 and above, against every 100 working-age individuals. A high dependency ratio means the productive population is supporting a larger non-working group, which strains pensions, healthcare budgets, and family resources.
India’s overall dependency ratio has been falling because of a shrinking child population, opening up what is often called the demographic dividend. The UNFPA notes that the dependency ratio fell from 75% in 2001 to 65% in 2011, and is projected to remain low for about two decades before climbing again. The catch is that the composition of dependency is changing. Even as the child dependency burden shrinks, the old-age dependency burden is steadily rising.
Old-age dependency: the slow-moving challenge
The old-age dependency ratio counts only the elderly against the working-age population. According to the National Statistical Office’s Elderly in India report, this ratio rose from 10.9% in 1961 to 14.2% in 2011. Projections suggest it will climb to 15.7% in 2021 and around 20.1% by 2031, a much sharper jump than the country experienced in the previous fifty years.
This matters for policy in concrete ways. Pension obligations expand, public health spending must reorient toward chronic and degenerative diseases, and labour markets need to plan for longer working lives. The UNFPA India Ageing Report 2023 highlights that more than 40% of the elderly fall in the poorest wealth quintile, and nearly 18.7% live without any income at all. Such levels of vulnerability mean the dependency ratio is not just a statistical figure but a measure of who will struggle to afford food, medicine, and dignity in old age.
Rising median age and a changing nation
Median age is the point that divides a population into two equal halves. It is one of the simplest ways to track how a society is ageing. India’s median age was around 22 years in 2001 and rose to about 24 years by 2011. The United Nations estimated the median age at 26.8 years in 2015, and current projections place it close to 28 to 29 years today. By 2036, it is expected to cross 35 years for the country as a whole.
Regional variations within the country
The national figure hides sharp differences between states. Kerala is currently India’s oldest state, while Bihar remains the youngest, with the median person in Kerala over a decade older than the median person in Bihar. By 2031, Tamil Nadu is projected to overtake Kerala with a median age of nearly 40 years. Southern and western states like Tamil Nadu, Karnataka, Andhra Pradesh, and Maharashtra have completed their fertility transition earlier and are ageing faster. Central and eastern states such as Bihar, Uttar Pradesh, Jharkhand, and Madhya Pradesh still have younger populations because of higher fertility.
This regional divergence has serious implications for welfare planning. A pension scheme designed around national averages may underfund Kerala’s needs while overprovisioning for Bihar. Healthcare infrastructure for the elderly, including geriatric wards, palliative care, and dementia services, must be planned state by state.
Gender disparities in ageing
One of the most striking patterns in census data is the growing predominance of women among the elderly. In simple terms, women live longer than men, and this gap widens with age. According to the National Statistical Office, the sex ratio for India’s elderly population is around 1,065 women per 1,000 men, and it is projected to rise further. The phenomenon, often called the feminisation of ageing, has profound consequences for policy.
Higher life expectancy, but harder lives
Indian women now have a life expectancy of about 73.6 years at birth, compared with 70.5 years for men, as cited in research drawing on the Longitudinal Ageing Study in India. The gap is even more pronounced at older ages in some states. The UNFPA reports that in Himachal Pradesh and Kerala, women aged 60 can expect to live another 23 and 22 years respectively, four years more than men in those states.
Yet longer life does not translate to better life. Older women report higher rates of chronic conditions, functional limitations, and women-specific health issues. The prevalence of depression is also higher among elderly women than men, with cognitive scores generally lower due to historic gaps in education and literacy.
Widowhood, poverty, and dependence
Marriage age gaps and longer female survival mean that around 54% of women aged 60 and above in India are widowed. Widowhood frequently brings with it loss of income, weakened property rights, and dependence on adult children, particularly sons. Many of these women spent their working years in unpaid domestic labour or informal employment, leaving them without pensions or personal savings. The UNFPA’s thematic paper on older women documents how awareness of pension schemes and government facilities is itself lower among elderly women than men.
This is why gender-sensitive policy matters. Universal old-age pensions, widow pensions with simpler documentation, dedicated geriatric outpatient services for women, and community-based caregiving programmes can address vulnerabilities that gender-neutral schemes often miss. Programmes like the Indira Gandhi National Old Age Pension Scheme and the National Programme for Health Care of the Elderly need to be calibrated to reach widowed women living alone in rural areas, where two-thirds of the elderly continue to reside.
Why the census data should drive proactive planning
Looking across the three censuses, a consistent story emerges. The elderly share has grown from around 6.7% in 1991 to 8.6% in 2011, and projections place it above 20% by 2050. The old-age dependency ratio is climbing, the median age is rising, and women dominate the upper age brackets while facing the steepest disadvantages. These are not separate trends but interlocking parts of the same demographic transformation.
The policy lesson is that India still has a narrow window. Unlike many developed nations that grew rich before they grew old, India is ageing while still grappling with poverty, weak social security coverage, and underdeveloped geriatric healthcare. Building sustainable pension systems, universal health coverage that includes elder care, and age-friendly cities and housing requires planning today for a population whose shape will be very different in twenty years.
The census also reminds us that ageing is not uniform. It is faster in the south and west, slower in the central and eastern states. It is harder for women than men, harder for rural elders than urban ones, and harder for those in the poorest wealth quintiles. A single national policy cannot address this diversity. Targeted, gender-sensitive, region-specific welfare frameworks are the only way forward.
