India’s population story is one of the most dramatic demographic journeys in human history. From a country of just over 238 million in 1901 to a nation that crossed 1.4 billion and overtook China as the world’s most populous country in 2023, the trajectory has been anything but linear. Understanding how this growth unfolded, why it accelerated, and why it is now visibly slowing reveals a lot about public health, agriculture, education, and the lived choices of ordinary families. The pre-1921 era: stagnation, fluctuation, and high mortality
In the early decades of the 20th century, India’s population grew at a crawl. Between 1901 and 1921, the average annual growth rate stayed below 1%, with some decades even showing negative growth. The reason was not low fertility. Women were having large numbers of children, but a near-equal number were dying young. This pattern fits what demographers call Stage 1 of the Demographic Transition Model: high birth rates matched by high death rates.
Several forces kept mortality stubbornly high. Recurring famines, infectious diseases like cholera, plague, malaria, and smallpox, poor maternal and child healthcare, and limited sanitation infrastructure meant that life expectancy hovered around just 25 years. The 1911-1921 decade stands out as the only census period in India’s recorded demographic history when the population actually declined, falling by about 0.31% over ten years.
Why 1921 is called the ‘Great Divide’
The 1921 census marks what demographers and historians universally refer to as the Year of the Great Divide. It separates two fundamentally different demographic regimes in India. Before 1921, growth was erratic, slow, and occasionally negative. After 1921, growth became steady, positive, and eventually rapid. The decade-by-decade pattern is striking: the 1911-1921 period showed negative growth, while the 1921-1931 decade recorded a sharp turnaround with population beginning to expand consistently.
What caused this shift? It was not a sudden drop in birth rates. Fertility remained high. Instead, death rates began to fall as colonial public health interventions started taking effect. Progressive control of epidemics like cholera, plague, and malaria, the rollout of basic vaccination, improved railway networks that helped move food during shortages, and early sanitation reforms gradually reduced mortality. India was entering Stage 2 of the demographic transition, with declining death rates but still-high birth rates, the classic recipe for accelerating population growth.
Famines and epidemics: events that shaped the curve
The pre-1921 demographic story cannot be told without understanding the catastrophic mortality events that suppressed population growth. These were not background details. They were the dominant force shaping the curve.
The 1918 influenza pandemic
The single most devastating demographic event in 20th-century India was the 1918 influenza pandemic, sometimes called the Bombay Fever. It is estimated to have killed between 12 and 18 million people in India alone, which was roughly 5% of the total population at the time. India suffered the largest absolute death toll of any single country during the pandemic, with the highest percentage of excess deaths in the world at 4.39%.
The flu arrived through the port city of Bombay (now Mumbai) in mid-1918 and spread along railway routes to the rest of the country. Death rates peaked between September and November 1918. The Central Provinces alone reported 791,000 deaths in just two months. The pandemic disproportionately killed young adults, including women of reproductive age, which had long-term consequences for fertility and family structures. Studies show that population density and rail connectivity were strong predictors of mortality, with the north and central regions hit hardest.
Famines that altered demographic trajectories
Beyond the 1918 flu, recurring famines repeatedly slowed population growth. The Great Famine of 1876-1878 killed an estimated 5.5 million people across southern and western India. The Bengal Famine of 1943 remains one of the most discussed famines in modern history, claiming approximately 2-3 million lives. As economist Amartya Sen, who witnessed the famine as a child, has argued, the Bengal famine was not caused by a shortfall in food production but by an entitlement failure, where wartime inflation, hoarding, and disrupted distribution prevented the poor from accessing available food. It is widely regarded as the last major famine on Indian soil.
These mortality crises shaped not just total population numbers but also the age structure, sex ratios, and regional distribution of the population, leaving demographic echoes that lasted for decades.
The post-1951 population explosion
If 1921 was the Great Divide, the years after 1951 marked what is widely called the population explosion. The first post-independence census in 1951 recorded a population of about 361 million. By 1981, that number had nearly doubled to 683 million, an increase of close to 89% in just three decades.
This was the steepest period of growth in Indian demographic history. Annual growth rates climbed to between 2.0% and 2.2%, peaking at around 2.22% during the 1971-1981 decade. To put this in perspective, India was adding roughly the population of a medium-sized country every single year. The drivers were clear and well-documented in demographic literature.
Why mortality fell so fast
Independent India invested heavily in public health. Death rates dropped from about 27 per 1,000 in 1951 to around 15 per 1,000 by 1981. Life expectancy nearly doubled, climbing from about 32 years to 54 years over the same period. Several factors converged to make this possible: the rollout of primary health centres in rural areas, mass vaccination campaigns that eradicated smallpox by 1977, antibiotics and antimalarial programmes, and the spread of basic sanitation. The Green Revolution of the late 1960s improved food security dramatically, ending the cycle of famine-driven mortality that had defined earlier centuries.
Why fertility stayed high
Birth rates, however, fell only modestly during this period, from about 40 per 1,000 in 1951 to roughly 33 per 1,000 by 1981. Although India launched the world’s first official family planning programme in 1952, fertility remained high because of deep-rooted social factors: early marriage, low female literacy, preference for sons, the economic value of children in agricultural households, and limited access to contraception. The gap between falling death rates and slow-falling birth rates is precisely what produces rapid population growth. India was now firmly in Stage 2 of the demographic transition, with the population doubling roughly every 30 years.
The slowdown after 1981: the second great divide
Some demographers argue that 1981 deserves to be called another year of great divide, because it marks the point where India’s growth rate began a clear and sustained decline. While the absolute population kept climbing, the annual growth rate decreased steadily from about 2.22% in the 1970s to around 1.6% by 2011, and is now below 1%.
The drivers of this slowdown are interconnected and reflect deep socio-economic change. Female literacy rose dramatically, from about 9% in 1951 to over 70% today. Urbanisation accelerated, with more than 35% of Indians now living in cities and towns. Age at marriage rose. Contraceptive use spread, with modern method usage climbing from 47.8% in 2015-16 to 56.5% in 2019-21. Infant mortality fell sharply, reducing the perceived need for large families as insurance against child deaths.
Crossing replacement-level fertility
The most striking recent milestone came with the National Family Health Survey-5 (NFHS-5), conducted between 2019 and 2021. It revealed that India’s Total Fertility Rate (TFR), the average number of children born per woman, had fallen to 2.0, dropping below the replacement level of 2.1. This is a historic threshold. It means the average Indian woman is now having fewer children than needed to replace her generation.
The urban-rural divide is telling. Urban India has a TFR of about 1.6, while rural India sits at around 2.1. According to official government data, 31 states and union territories, accounting for nearly 70% of the country’s population, have already achieved fertility rates below replacement level. States like West Bengal, Tamil Nadu, Kerala, Andhra Pradesh, and Karnataka have TFRs comparable to many developed nations.
Modern drivers of declining fertility
Several reinforcing factors explain why birth rates have dropped so consistently. Women with 12 or more years of schooling have a TFR of just 1.7, compared to 2.8 for women with no formal education. Urban lifestyles, the rising cost of raising children, the cost of education, and changing aspirations have all pushed couples toward smaller families. Greater female workforce participation has delayed marriage and first births. Government initiatives like Mission Parivar Vikas have improved access to family planning services in high-fertility districts.
Regional asymmetry in the transition
The national picture hides sharp regional variation. While southern and western states have moved deep into the late stages of demographic transition, several northern and eastern states, including Bihar, Uttar Pradesh, Jharkhand, Meghalaya, and Manipur, still have TFRs above replacement level. This regional divergence reflects gaps in female literacy, healthcare access, urbanisation, and economic development. It also means India’s population will keep growing in absolute terms for the next two to three decades due to population momentum, even as fertility falls, because of the large cohort of young women now entering reproductive age.
What the journey reveals
India’s demographic story, from stagnant growth before 1921 to explosive expansion after 1951 to today’s gradual slowdown, mirrors the country’s larger journey through public health milestones, agricultural transformation, social change, and economic development. The Great Divide of 1921 was driven by declining mortality. The post-1951 explosion was driven by even sharper mortality declines without matching fertility declines. The current slowdown is being driven by something fundamentally different: changes in what families want, what women aspire to, and what economic life demands. The next chapter, an ageing population and the challenges of a shrinking workforce, is just beginning to unfold.
