Nutrition security is not the same as having a full stomach. It means having reliable access to food that meets all the body’s needs for energy, protein, vitamins, and minerals to grow, work, and resist disease. India has made remarkable strides in producing grain, yet the country still carries one of the heaviest burdens of malnutrition in the world. The reasons go far beyond empty plates. From a warming atmosphere that strips nutrients out of rice grains, to families where daughters and mothers eat last, the obstacles are tangled together. Understanding these issues is the first step toward solving one of the most stubborn public health challenges of our time.

Climate change: The invisible hand on our plates

The connection between a hotter planet and a hungrier nation runs deeper than most people realise. Climate change is reshaping what grows in our fields, how much it yields, and even how nourishing the harvest actually is.

How rising CO2 thins out our staple foods

One of the most counter-intuitive findings of recent nutrition science is that the very gas driving global warming also weakens the nutritional value of crops. Laboratory and field studies show that higher atmospheric CO2 concentrations reduce the protein and micronutrient content of wheat, rice, and legume crops. This means that even when a farmer harvests the same quantity of rice as a decade ago, each grain may carry less iron, zinc, and protein. For a country where cereals provide nearly half of all protein intake in rural areas, this dilution effect is alarming.

Disrupted agriculture and unstable yields

Climate change affects nutrition through agriculture in several ways. Temperature variations, erratic rainfall, and extreme weather events disrupt crops, livestock, poultry, and aquaculture production, leading to food shortages, income loss, and elevated food prices. The crops worst hit are often the ones richest in nutrients. A study by ICRISAT and the World Food Programme found that 65 to 87 percent of districts growing maize, pigeon pea, chickpea, sorghum and sesame show high production instability due to climate variability. These are precisely the pulses, millets and oilseeds that provide the protein, micronutrients and healthy fats missing from a rice-and-wheat-only diet.

Why the poorest are hit hardest

Climate-linked nutrition risks are not distributed evenly. Rainfed agricultural districts, where farmers depend entirely on the monsoon, suffer the steepest yield losses. These districts also tend to be home to India’s poorest communities. When prices of pulses, vegetables, and oilseeds spike after a bad season, low-income households are forced to cut back on the very foods that fight malnutrition. Climate change, in this sense, is not just an environmental issue but a deepening engine of inequality.

If you ask whether Indians today eat more or less than their parents did, the answer is surprisingly mixed. The data tells a story of slow shifts that conceal serious problems.

The long-term decline and recent stabilisation

Successive rounds of the National Sample Survey have tracked Indian diets for decades. According to historical data, there has been a slow but steady decline in consumption of calories and proteins, while the consumption of fat has been increasing both in rural and urban India. The most recent Household Consumption Expenditure Survey shows that average daily per capita calorie intake in rural India stood at 2,233 kilocalories in 2022-23, slightly decreasing to 2,212 kilocalories in 2023-24, while protein intake has remained stable at around 62 grams per day. The intake numbers look reasonable on paper, but the composition of these calories has changed in ways that matter.

The shifting sources of protein

Cereals continue to be the main source of protein for most Indians, but their dominance is fading. The share of cereals in total protein intake has declined sharply over the past decade, by about 14 percent in rural India and nearly 12 percent in urban India, with this gap being filled by eggs, fish, meat, and dairy. This sounds like progress, and for middle-income households it is. But the same survey reveals a troubling counter-trend: protein intake has dropped significantly among the poorest rural households. The dietary transition is bypassing those who need it most.

Hidden hunger: When the belly is full but the body is starved

The biggest blind spot in measuring nutrition security is hidden hunger, the silent deficiency of essential vitamins and minerals. A person can eat enough rice every day and still suffer from severe iron, vitamin A, or zinc deficiency. The numbers in India are staggering. According to UNICEF, more than 80 percent of adolescents in India suffer from hidden hunger, with one in every two adolescents deficient in at least two of six key micronutrients.

Anemia, the most visible signal of hidden hunger, has actually worsened in recent years. According to NFHS-5, nearly one in two Indian children aged 6 to 59 months is anaemic, with many also suffering from concurrent deficiencies of iron, folate, vitamin B12, and other micronutrients. The condition strikes hardest at women and children, undermining pregnancy outcomes, foetal neurodevelopment, and lifelong cognitive potential. Hidden hunger does not produce the dramatic images of starvation we associate with food crises, but it slowly drains a nation’s health and human capital.

The double burden of malnutrition

India now faces what nutritionists call the double burden: undernutrition and obesity often exist in the same village, sometimes within the same household. As fat intake rises and physical activity declines, lifestyle diseases like diabetes are climbing even as anemia and stunting persist. Achieving nutrition security today means tackling both ends of this spectrum simultaneously.

Social and gender dynamics: The hidden architecture of who eats what

Nutrition is shaped by far more than what is available in the market. Inside the home, decisions about who gets the egg, who drinks the milk, and who waits till everyone else is done are governed by deep-rooted social rules.

Women last and least: The cost of serving order

Across much of India, especially in rural and conservative households, women eat after the men and boys of the family have finished. Studies document that in states like Bihar, Jharkhand and Maharashtra, it is common for women to eat only after all men have finished. When food is plentiful, this may seem like a harmless tradition. When supplies are tight, or when the family is poor, it means women systematically receive smaller portions and lower-quality food.

The biological irony is sharp. Women have higher nutritional needs during menstruation, pregnancy, and lactation, yet they consistently receive less. These physiological demands, if unmet, exacerbate deficiencies such as anaemia and underweight status, impacting both maternal and child health. The result is visible in the data: more than half of Indian women in the reproductive age group are anaemic, and the rates have actually risen between NFHS-4 and NFHS-5.

Food taboos during pregnancy and lactation

Pregnancy is the most nutritionally demanding phase in a woman’s life, yet it is also the period most heavily policed by cultural restrictions. A study of perinatal nutrition in rural Bihar found that 85 percent of mothers and 75 percent of ASHA workers did not increase dietary intake during pregnancy, contrary to medical guidelines. The same study showed that traditional classifications of foods as hot or cold, with nutritious items like eggs and papaya feared as causing miscarriage, continue to dictate what a pregnant woman is allowed to eat.

The list of restricted foods often includes the most nutrient-dense options available, such as green leafy vegetables, eggs, fish, and certain fruits. The combination of taboos, gender norms, and economic constraints creates what researchers call a cluster of barriers including food unavailability, economic constraints, low exposure to nutrition counselling, food restrictions and taboos during pregnancy, and gaps in maternal knowledge.

Social status, caste, and access to nutrition

Nutrition security is also shaped by social hierarchies that determine which households can afford diverse diets, which neighbourhoods have functioning Anganwadi centres, and which children are reached by school meal programmes. Marginalised communities, including Dalits, Adivasis, and migrant labour households, consistently show poorer nutrition outcomes. The interacting social, economic and agricultural drivers of India’s micronutrient deficiencies include relative food pricing of energy-dense versus micronutrient-rich produce, farmer income effects, and inequitable gender access. Climate shocks, food inflation, and weak welfare delivery hit these groups first and hardest.

Women’s empowerment as a nutrition solution

The flip side of these social constraints is an opportunity. When women have greater control over household resources, both they and their families tend to be healthier and better nourished, because women tend to spend more on the nutrition, health, and well-being of their households when they have decision-making authority. This is why interventions that improve girls’ education, delay early marriage, and strengthen women’s earnings have such powerful spillover effects on child nutrition. Tackling gender inequality is not separate from tackling malnutrition; it is the same battle.

Why these issues must be tackled together

Climate change, declining diet quality, hidden hunger, and gender inequity are not separate problems. They reinforce one another. A bad monsoon raises pulse prices, pushing poor households back to rice-only meals. The reduced diversity worsens hidden hunger, especially in women whose intake is already constrained by social norms. Their underweight, anaemic babies grow into adults with lower productivity and higher disease burden. Solutions that focus on only one piece of this puzzle, whether more grain in the ration shop or a single fortified product, will keep missing the larger picture.

Achieving real nutrition security requires climate-resilient agriculture that prioritises millets, pulses, and oilseeds; food systems that move beyond cereals to encourage dietary diversity; targeted programmes that reach adolescent girls, pregnant women, and the poorest rural households; and a steady chipping away at the social norms that decide who eats first and who eats last.
