Every researcher in social science eventually faces the same practical question: how do you actually measure what people know, recall, or understand? Whether you are studying health awareness in a rural community or testing comprehension of family planning concepts among college students, the design of your test questions decides whether your data is clean or muddled. Objective test questions are the workhorses of this measurement task. They are quick to administer, easy to score, and when written well, surprisingly powerful tools for capturing what respondents truly know.

What objective test questions are

Objective test items are questions with answers that are either correct or incorrect, leaving no room for the scorer’s personal judgment. According to the Center for Innovation in Teaching and Learning at the University of Illinois, objective items include multiple-choice, true-false, matching, and completion formats, while subjective items cover essays and extended responses. The defining feature is consistency: two different graders evaluating the same answer will arrive at the same result.

In population and family health studies, this reliability matters enormously. Surveys on contraceptive knowledge, nutrition awareness, or maternal care often involve thousands of respondents. Subjective scoring at that scale introduces bias and slows analysis. Objective items, by contrast, allow researchers to assess a wide range of textual and numerical responses while keeping scoring fast and fair. The trade-off is that these questions lean toward measuring recall and recognition rather than synthesis or creativity, so researchers must choose formats deliberately.

True or false questions

True or false questions present a statement, and the respondent decides whether the proposition is correct. They are the simplest objective format and arguably the most underestimated. A single true or false item takes seconds to answer, which means a researcher can sample a large amount of subject matter in a short questionnaire. This makes them especially useful in community health surveys where respondent fatigue is a real concern.

When true or false works best

Use this format when you want to measure recall of factual information or basic comprehension of well-defined concepts. For example, a statement like “ORS solution is used to treat dehydration during diarrhoea” yields a clean yes or no response. Because the cognitive load is low, true or false items are excellent for screening baseline knowledge before introducing more demanding items.

Guidelines for writing them well

The biggest enemy of a true or false item is ambiguity. The statement must be unambiguously true or unambiguously false; partially correct propositions ruin the format. The Handbook of Learner Evaluation and Test Item Construction recommends keeping each proposition short, focused on a single idea, and free of qualifiers that telegraph the answer. Words like always, never, all, and none often signal a false statement, while usually, sometimes, and generally often signal a true one. Mix the structure of true and false items so the respondent cannot guess based on length or phrasing, and avoid double negatives that force the reader to untangle grammar before reasoning about content.

Multiple choice questions

Multiple choice items, often called MCQs, are the most flexible objective format. Each item has a stem (the question or incomplete statement) and a set of options, of which one is the correct answer and the others are distractors. MCQs can measure factual recall at the basic level, but with careful construction they can also evaluate application and analysis. This versatility is why they dominate competitive exams in India, from UPSC preliminary papers to national medical entrance tests.

Anatomy of a good MCQ

A strong MCQ begins with a stem that poses a self-contained question. The University of British Columbia teaching guidelines suggest that the stem should be answerable even before the options are read, with simple sentence structure and minimal negative phrasing. The intended answer must be indisputably correct, and the distractors should be plausible enough to challenge respondents who only partially understand the concept.

Common pitfalls in distractor design

Writing plausible distractors is the hardest part of MCQ construction. Distractors that are obviously wrong reduce the question to a true or false item with extra steps. The Center for Innovative Teaching and Learning at Indiana University Bloomington warns against making the correct option noticeably longer, shorter, or more grammatically polished than the distractors, since this leaks the answer. The phrase all of the above should be avoided, and none of the above should be used with caution. Options should be parallel in structure and grammatically consistent with the stem.

For surveys in family health research, a practical rule from survey design literature drawing on Pew Research and US Census Bureau guidance is to offer four to seven options for single-select items. Fewer options invite guessing; too many overwhelm respondents and pad response time without improving measurement.

Fill in the blanks questions

Fill in the blanks items, sometimes grouped under the broader category of completion questions, present a sentence with one or more words removed. The respondent supplies the missing term. These items belong to the constructed-response family, meaning the test-taker must produce the answer rather than recognise it.

Why they test deeper recall

Fill-in-the-blank items demand active recall, not recognition. The American Journal of Pharmaceutical Education notes that constructed-response items like fill-in-the-blank may provide a more accurate assessment of knowledge because the examinee must construct or synthesize their own answer rather than selecting one from a list. This makes them particularly useful when guessing would distort results, such as in pre-test and post-test designs measuring the impact of a health education intervention.

Construction guidelines

The cardinal rule is that each blank should have exactly one correct answer, or at most a small set of acceptable synonyms. Place the blank near the end of the sentence so the context builds toward the missing term. Keep blanks uniform in length so their physical size does not hint at the answer. Limit each item to one or two blanks; the more gaps in a sentence, the more it dissolves into guesswork. Avoid lifting sentences verbatim from textbooks, since this tests memorisation of phrasing rather than understanding. The grammar of the surrounding sentence should not betray the answer, so articles like a versus an need careful handling.

Matching questions

Matching items present two parallel columns and ask the respondent to pair each entry in one column with the correct entry in the other. They are efficient for testing relationships between two sets of related information, such as diseases and their causative agents, demographic indicators and their definitions, or policy schemes and their launch years.

Strengths of the matching format

A single matching block can cover many concepts in a compact space. For research on health policy literacy, for example, one matching item can simultaneously assess whether respondents can pair government schemes like Janani Suraksha Yojana, ICDS, and Ayushman Bharat with their respective focus areas. This compactness is why matching items are popular in classroom assessment and survey research alike.

Rules for writing matching items

The premises (the items to be matched, usually in the left column) and the responses (the options, usually in the right column) should belong to the same logical category. Mixing dates, names, and definitions in one response column makes the task trivial because elimination becomes too easy. Established guidance on test construction recommends including more responses than premises so that the final pairing is not solved by simple elimination. Keep the entire matching set on a single page to avoid confusion, give clear directions about whether a response can be used more than once, and limit each set to a manageable number of items, typically five to ten.

Completion questions

Completion questions are closely related to fill-in-the-blank items but are sometimes treated as a distinct category. In a completion item, the respondent finishes a statement that has been left incomplete, usually by supplying a word, phrase, or short numerical value. The distinction from fill-in-the-blank is subtle: completion items often place the missing element at the very end of the sentence, while fill-in-the-blank items may have blanks embedded mid-sentence.

What they measure

Completion items test comprehension and retention. They work well for who, what, when, and where questions, especially when the answer is a specific term, number, or date that the respondent should have memorised. In population studies, a question like “The infant mortality rate is the number of deaths of infants under one year of age per ________ live births” tests whether the respondent has internalised a precise demographic definition.

The iSpring guide on writing completion-style questions recommends ensuring the sentence still makes sense without the missing term and that the intended answer is easily derivable from context. The blank should target a single, important concept rather than a trivial word. Avoid filling a sentence with so many gaps that the surrounding text loses meaning. Scoring should account for predictable variations such as synonyms, spelling, and capitalisation, particularly when responses are graded manually or by simple automated systems.

Choosing the right mix for social science research

No single objective format is universally superior. Each tests a different facet of knowledge and carries its own limitations. True or false items sample content broadly but invite guessing. MCQs measure a range of cognitive levels but require careful distractor design. Fill-in-the-blank and completion items reduce guessing but limit content coverage and complicate scoring. Matching items efficiently test relationships but only work for content that comes in natural pairs.

Best practice in examination design, as summarised by research on item construction and post-hoc review, is to use a mixed approach that draws on multiple item types within the same instrument. A questionnaire on adolescent reproductive health awareness, for example, might open with true or false items to establish baseline recall, move into MCQs to test conceptual understanding of contraceptive methods, and close with a few fill-in-the-blank items to verify deeper retention of key terms. This blend balances coverage with depth and protects the data against the weaknesses of any single format.

The clarity of wording across every item type is non-negotiable. Ambiguous statements, leading distractors, and grammatical clues all introduce measurement error that no amount of statistical analysis can repair. Pilot testing the instrument on a small group representative of the target population is the cheapest insurance against these flaws.
