The world is on the cusp of a demographic shift unlike anything in recorded history. For the first time, scientists can confidently predict a peak to human population growth within our lifetimes, followed by a slow decline. The latest United Nations projections suggest that by the mid-2080s, the world will reach its highest-ever headcount of roughly 10.3 billion people, before settling at around 10.2 billion by 2100. What does this mean for healthcare systems, migration corridors, climate goals, and the very structure of societies? The answer is more nuanced than the headlines suggest.

The big numbers: What the UN actually projects

Every two to three years, the United Nations Population Division releases its World Population Prospects, the most authoritative global dataset on population trends. The 2024 revision, drawing on data from 1,910 censuses across 237 countries and areas, paints a picture that is both reassuring and challenging.

Global population stood at 8.2 billion in 2024. According to the medium-variant projection, this figure will cross 9 billion by 2037, reach 10 billion around 2061, and peak at approximately 10.3 billion in the mid-2080s before gradually declining to 10.2 billion by 2100. That decline at the end of the century is small in absolute terms but historically momentous. It marks the close of a centuries-long era of human expansion.

To put this in perspective, the global population was just 2.6 billion in 1950. Within a single human lifespan, we will have nearly quadrupled our numbers and then begun to taper off. The size of the world’s population in 2100 is now expected to be about 700 million smaller than projections a decade ago anticipated, largely because fertility has dropped faster than expected in many large countries.

The assumptions behind the projections

UN projections are not crystal-ball predictions. They are probabilistic models built on three demographic levers: fertility, mortality, and migration. The “medium variant” represents the median outcome of thousands of simulations. High and low variants explore what happens if fertility rates run half a child higher or lower per woman than the medium scenario.

Globally, fertility has fallen from nearly 5 children per woman in the 1960s to about 2.25 today. The UN expects this rate to fall to the replacement level of 2.1 by 2036, and continue dropping after. More than half of all countries already have fertility below replacement level, including China, Japan, South Korea, Germany, Italy, and the United States. India joined this list recently, with a Total Fertility Rate of 1.9 births per woman, below the replacement threshold of 2.1.

Where the growth will come from

Not all regions are on the same demographic curve. Population growth between now and 2100 will be highly concentrated. Five countries are expected to contribute more than 60 percent of the world’s population growth by 2100: the Democratic Republic of Congo, Ethiopia, Nigeria, Pakistan, and Tanzania.

Sub-Saharan Africa stands apart. Its population is expected to nearly triple, from 1.2 billion today to 3.4 billion by 2100. By the end of the century, Nigeria and the DRC are projected to be the fourth and fifth most populous countries on Earth.

India’s trajectory

India became the world’s most populous country in 2023 and will hold that position through 2100. The UN projects India’s population to peak at roughly 1.7 billion in the early 2060s before declining by about 12 percent to around 1.5 billion by 2100. Currently, 68 percent of India’s population is in the working-age group of 15 to 64, offering what economists call a demographic dividend, provided employment and skill-building policies can match the moment.

This dividend, however, is not evenly distributed across states. Southern states like Kerala and Tamil Nadu have fertility rates well below replacement and are aging rapidly, while several northern and eastern states still have higher fertility. The result will be very different demographic challenges within the same country over the coming decades.

The great inversion: An aging world

The most consequential shift of the century is not the peak itself but the changing age structure that comes with it. The world is aging at a pace humanity has never seen.

The global median age is expected to rise to 42 by 2100, up from 31 today and just 22 in 1950. The number of people aged 65 and older will more than double from 857 million today to 2.4 billion by 2100, raising their share of the global population from 10 percent to 24 percent. By the late 2070s, the UN projects there will be more people over 65 than children under 18 worldwide.

Europe’s demographic dilemma

Europe is the oldest region on Earth. Its median age has already crossed 42, and roughly 20 percent of Europeans are 65 or older. In the European Union, the labour force is expected to fall from 251 million to 228 million by 2050, while the number of people over 65 will rise from 112 million to 142 million. The EU’s share of the global population is set to shrink from 6 percent to just 4 percent by 2100.

Africa, in stark contrast, has a median age of just 19. This gap is the demographic fault line of the 21st century.

Migration: The pressure valve of the 21st century

Aging societies need workers. Younger societies often need opportunity. The mismatch is creating one of the most powerful migration drivers in modern history.

The UN now projects that immigration will be the main driver of population growth in 52 countries through 2054 and 62 countries through 2100, including Australia, Canada, and the United States. Without immigration, the populations of many wealthy countries would shrink much faster than they already are.

The push and pull factors

On the push side, climate change is already displacing people from parts of the Horn of Africa, West Africa, and South Asia. Lack of decent jobs and the gap between population growth and economic growth in many low-income countries adds pressure. On the pull side, declining workforces in Germany, Japan, South Korea, China, and parts of Europe create gaps that cannot be filled by domestic births or higher retirement ages alone.

Already in 2022, Europe had six million vacant jobs. Analysts at organisations like the Friedrich Naumann Foundation argue that the EU and UK will be short tens of millions of workers by 2050, a deficit unlikely to be closed by higher birth rates or longer working lives alone.

For India, this raises a strategic question. As Indian fertility falls and the country gradually ages after its peak in the 2060s, will India become a destination for workers from younger neighbours, or continue as a source of skilled migration to aging economies? The answer will depend heavily on how well India invests in education, healthcare, and job creation for its current youth bulge.

Implications for global health

An older world is a sicker world in some ways, and a healthier one in others. Life expectancy at birth has reached 73.3 years globally in 2024 and is projected to rise to 77.4 years by mid-century. That is something to celebrate. But it shifts the disease burden dramatically toward chronic conditions like diabetes, dementia, cardiovascular disease, and cancer, all of which are more expensive to manage than acute infections.

Healthcare spending in the EU alone is projected to rise from 24.6 percent of GDP in 2019 to nearly 27 percent by 2040, largely driven by long-term care and pensions.

The reproductive health agenda

At the other end of the spectrum, early childbearing remains a serious challenge. In 2024, 4.7 million babies were born to mothers under age 18, with about 340,000 born to girls under 15. Investing in girls’ education, raising the age of marriage, and expanding access to family planning would not only improve maternal and child health but also slow population growth in the regions that need it most.

The sustainability question

A world of 10 billion people will need food, water, energy, and shelter, all within a climate that is itself changing rapidly. The good news, according to UN demographers, is that an earlier and lower peak in global population could mean reduced environmental pressures from lower aggregate consumption over the long term.

The harder truth is that population size is only one variable. Consumption patterns, particularly in wealthy and rapidly developing economies, matter just as much. Most of the countries with continuing population growth, like those in Sub-Saharan Africa, currently have very low per-capita emissions. If they industrialise along the same fossil-fuel-heavy path that today’s rich countries took, the climate math becomes very difficult.

This is why population, development, and climate policy are increasingly seen as one interconnected challenge. Stabilising fertility through voluntary family planning and education, paired with cleaner growth pathways for emerging economies, may be the most realistic route to a sustainable 22nd century.

What this means for the next generation

For students entering universities today, the world of 2050 and 2100 will not just be more populous in some places and less in others. It will be older, more urban, more migratory, and more interdependent. Healthcare systems will need to be redesigned around chronic disease and elder care. Pension systems built on the assumption of an expanding workforce will require fundamental reform. Cities in Africa and South Asia will absorb the bulk of new urban dwellers, requiring massive investment in infrastructure and climate-resilient housing.

The countries that thrive will likely be those that prepare early: investing in education and skills, building flexible labour markets, planning humane migration policies, and shifting healthcare from sickness-treatment toward prevention and long-term care.
